Volunteer Update Form

If you are an active volunteer with Friend for Life, we'd love to know what's new with you. Most importantly, we want to know how you are, health-wise, so that we can be of support to you and so that we can continue to make the most appropriate matches with callers. But, we also love to hear about the rest of your life — upcoming weddings, new grandchildren, travel plans.
